Job summary

Affinis Hospice is seeking a dedicated Certified Nursing Assistant to provide compassionate personal care to hospice patients in their homes. The role emphasizes comfort, dignity, and adherence to care plans under physician guidance.

You will support daily living activities, assist with basic housekeeping essential to patient health, document care provided, and promptly report any changes to the supervising nurse. Team collaboration is required.

Qualifications

• Completion of state-recognized and federally compliant training program and/or competency evaluation and at least six-month experience as a Certified Nursing Assistant.

Responsibilities

• Provides personal care to hospice patients as prescribed by the physician and in compliance with regulations.
• Promotes patient comfort and dignity and assists with daily living activities.
• Assists with household tasks essential to patient health and safety.
• Documents services accurately and submits records promptly.
• Reports changes in patients' conditions to the primary nurse/director.
• Participates in interdisciplinary team meetings and follows the patient care plan.
