Job summary

St. Croix Hospice is seeking an RN Clinical Supervisor to lead and coordinate patient care across the hospice team. You will collaborate with clinical leaders to ensure care plans are implemented and compliant with regulations.

The role requires an RN license, ADN with preferred BSN, and experience in interdisciplinary settings. On-call duties and ongoing staff development are part of the responsibilities. Competitive benefits are offered.

Qualifications

• Current RN license in state(s) of practice.
• Multistate license acceptable; practice in other states per PSOR.
• Graduate of accredited nursing program; ADN required.
• Ability to work in an interdisciplinary setting.
• Pass background, consumer report, and drug test.
• Valid driver's license and access to a reliable vehicle.
• Bachelor's degree preferred.
• 2+ years supervisory experience in hospice/homecare preferred.

Responsibilities

• Coordinate patient care and services across the hospice team.
• Review cases with Medical Director or Hospice Physician on eligibility.
• Assist in quarterly record reviews and quality improvement.
• Participate in on-call duties and interdisciplinary meetings.
• Mentor orientation for new employees.
