Job summary

St. Croix Hospice seeks an RN Clinical Supervisor to coordinate and deliver high-quality hospice care.

You will supervise care teams, ensure appropriate case management, and collaborate with the Medical Director and other clinical leaders to optimize patient outcomes. Responsibilities include reviewing cases, guiding clinicians, maintaining records in compliance with laws, and participating in ongoing staff development and quality improvement initiatives.

Qualifications

• Must hold a current RN license in state of practice (or multistate license).
• Graduate of an accredited school of nursing; associate degree required.
• Bachelor’s degree preferred; strong preference for hospice/homecare experience and supervisory background.

Responsibilities

• Coordinate patient care and services for hospice patients.
• Consult with Medical Director/hospice physician regarding eligibility and plans of care.
• Supervise care teams and ensure timely, compliant documentation and records.
