Job summary

St. Croix Hospice in Oakdale, MN is seeking an experienced RN Clinical Supervisor to coordinate and deliver hospice care aligned with each patient’s Plan of Care. You will collaborate with clinical leaders to manage care teams and ensure high-quality hospice services.

Qualifications

• Must have a current RN license in state(s) of practice.
• Multistate license can practice in other states.
• Graduate of an accredited school of nursing; Associate degree required.
• Proven ability to work within an interdisciplinary setting.
• Ability to pass background, consumer report, and drug test.
• Driving: valid license and vehicle in good standing.

Responsibilities

• Receives case referrals and assigns appropriate hospice personnel.
• Reviews and evaluates cases via home visits, conferences, and records.
• Available at all times during operating hours to assist clinicians.
• Reviews patient’s medical diagnosis, prognosis, medications, procedures, and clinical course.
• Assists clinicians in establishing immediate and long-term goals and plans of care.
• Attends case conference meetings to coordinate care.
• Performs quarterly record reviews and communicates findings to management.
• Assists in orientation of new employees.
• Maintains clinical records in compliance with laws.
• Supports development and evaluation of education programs and policies.
• Participates in quality assessment and performance improvement activities.
