Job summary

St. Croix Hospice is seeking an experienced RN Clinical Supervisor to coordinate and oversee patient care in a home-based hospice setting.

You will lead care teams, collaborate with clinicians and the Medical Director, and ensure plans of care align with each patient’s goals. The role requires an active RN license, an accredited nursing degree, and a track record in interdisciplinary environments; bachelor’s degree and 2+ years of supervisory experience are strongly preferred.

Qualifications

• Current RN license in state(s) of practice.
• Multistate license allowed for practice in other states.
• Graduate of an accredited school of nursing; associate degree required.
• Bachelor’s degree preferred; 2+ years supervisory experience preferred.

Responsibilities

• Coordinate and supervise patient care, ensuring services are delivered consistently.
• Assign hospice personnel to cases and consult with Medical Director as needed.
• Review cases via home visits, conferences, and record checks to guide care quality.
• Attend interdisciplinary meetings and participate in quality improvement activities.
• Maintain compliant clinical records and support orientation for new staff.
• Participate in on-call duties per policy and assist in plan of care development.
