Job summary

Anchor Health, a premier hospice provider, seeks an experienced RN Case Manager to coordinate home-based hospice care for patients and families in the Tracy, California area. The role focuses on developing and directing patient care plans, managing symptoms, and delivering compassionate education to support comfort and quality of life.

The RN must hold a California license, CPR certification, a valid driver’s license, reliable transportation, and willingness to travel to patient locations.

Qualifications

• Registered Nurse with CA state license.
• Hospice experience preferred.
• CPR certification current.
• Valid driver’s license with reliable transportation.
• Excellent interpersonal skills and compassion for patients and families.

Responsibilities

• Provide care to patients diagnosed with terminal illness.
• Regularly review and update the plan of care.
• Administer prescribed medications and manage pain/symptoms.
• Conduct initial and ongoing assessments of patient and family needs.
• Develop and implement care plans with patient/family involvement.
• Coordinate patient care as Case Manager within the interdisciplinary team.
• Document clinical notes and changes in patient condition.
• Communicate with physicians and community providers to coordinate care.
