Qualifications

• RN with Nursing Diploma or Associate degree.
• Preferred Bachelor’s Degree in Nursing or Home Health.
• 2–5 years experience in nursing or case management preferred.

Responsibilities

• Assess patient preferences, barriers, and support needs.
• Develop, implement, and document individual care plans with goals.
• Collaborate with interprofessional team and family for plan delivery.
• Promote equity and empathetic care across diverse populations.
• Coordinate care across continuum and update EMR as changes occur.
