Qualifications

• RN licensure in good standing in Missouri.
• Experience in hospice/home health preferred.
• Strong communication, collaboration and care coordination skills.

Responsibilities

• Assess patient preferences and barriers to involvement in care.
• Develop, implement and document plans of care with goals.
• Promote respect, equity and empathy in diverse patient populations.
• Communicate changes in plan of care with family and interprofessional team.
• Maintain safety and comply with regulatory requirements as a safety-sensitive role.
