BJC Home Care provides skilled nursing, hospice and home health services across Illinois. The RN Case Manager coordinates care for patients and families, guiding treatment plans and ensuring seamless communication among the care team.

This role requires a Nursing Diploma/Associate's, an active RN license, 2–5 years in home health, and willingness to participate in on-call and holiday rotations.

Qualifications

• Nursing Diploma/Associate's is required.
• RN license must be active.
• Experience in home health or hospice preferred.

Responsibilities

• Assess patient preferences and barriers to involvement in care.
• Develop and document individualized care plans with defined goals.
• Promote equity and empathy in care across diverse populations.
• Communicate plan changes to family and interprofessional team.
• Ensure care is evidence-based and documented in EMR.
• Safety-sensitive position; maintain alertness and safe practice.
