Job summary

Heartland Hospice in Illinois is seeking a Registered Nurse Case Manager to lead compassionate, home‑based care for patients facing life‑limiting illness. You will perform comprehensive assessments, develop personalized plans of care, coordinate with the interdisciplinary team, educate families, and ensure timely documentation and regulatory compliance.

Qualifications

• RN licensed in state of employment.
• Minimum 1 year RN experience in acute care, oncology, hospice or home health.
• Hospice or palliative care experience preferred.
• CPR certification required.
• Valid driver’s license and auto insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits; provide education to patients, families and caregivers.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized care plans with visit frequency.
• Coordinate care across interdisciplinary teams and consult with physicians.
• Educate patient/caregivers about disease process, end-of-life care, medications and nutrition.
• Document patient condition changes to relevant providers; ensure timely interventions.
• Maintain accurate clinical records and comply with infection control standards.
• Support continuity of care with home health, palliative and hospice referrals.
• Participate in on-call and patient support activities as applicable.
