Job summary

BJC Home Care is seeking a registered nurse case manager to support hospice services across the Illinois service area. This role focuses on coordinating comprehensive patient care, collaborating with a multi-disciplinary team, and supporting patients and families in their home or facility settings.

Candidates should hold an IL RN license and have 2–5 years of hospice/home health experience. A sign-on bonus and relocation options are available to eligible applicants.

Qualifications

• Registered Nurse in IL with valid license.
• Nursing diploma or associate degree required.
• 2–5 years of relevant hospice/home health experience.

Responsibilities

• Assess patient preferences and barriers to involvement in care, including values, spiritual and cultural needs.
• Develop, implement and document individualized plans of care with defined goals with the interprofessional team and family.
• Coordinate care across the continuum and communicate the plan to family or caregiver.
• Promote equity and empathy in care delivery and support emotional, cultural needs.
• Implement care plans, evaluate changes, update EMR and ensure evidence-based outcomes.
