Job summary

Heartland Hospice in Illinois is seeking a compassionate Registered Nurse to lead as an RN Case Manager, providing skilled home-based hospice care and coordinating with an interdisciplinary team. You will conduct patient assessments, develop and update Plans of Care, and support families with education and bereavement guidance.

• RN licensed in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or acute care.
• CPR certification is required.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home settings.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care across interdisciplinary teams and physicians.
• Educate patients and caregivers on disease process, end‑of‑life care, medications.
• Document timely clinical records and ensure regulatory compliance.
• Support on-call and patient support activities as applicable.
