BJC Home Care in the St. Louis area is seeking a Hospice - Registered Nurse Case Manager to coordinate patient care in home settings. Monday to Friday, 8:00am–4:30pm with weekend/on-call rotation.

You will evaluate patient needs, develop care plans with a multidisciplinary team, and ensure timely communication across the continuum, including EMR updates. The role supports professional growth via the BJC RN Career Ladder and may offer relocation for eligible candidates.

Competenze

• Nursing diploma/associate degree required for RN licensure.
• Bachelor’s degree in nursing or related home health preferred.
• Experience in home care or hospice settings is desirable.

• Assess patient preferences and barriers to involvement in care.
• Develop and document individual care plans with the interprofessional team.
• Communicate the plan of care across the continuum and update the EMR.
