BJC Home Care is seeking a Hospice - Registered Nurse Case Manager in the St. Louis area. Shift is Monday–Friday, 8:00am–4:30pm with weekend and on-call rotation. The role supports patients and families, providing end-of-life care in home or SNF settings.

The RN must coordinate care across the interprofessional team, document plans of care, and participate in BJC's RN Career Ladder and professional development offerings. Relocation and a sign-on bonus are available for eligible candidates.

Qualifications

• Evaluates the client and furnishes services requiring substantial and specialized skill, appropriate preventive and rehabilitative nursing procedures.
• When assigned as case manager, coordinates all aspects of care related to that patient.
• Works with interprofessional team to develop and implement care plans.

Responsabilités

• Assess patient preferences and barriers to involvement in care, including values, emotional, spiritual, cultural, and population-specific needs.
• Develops, implements, and documents individual plans of care with defined goals in collaboration with the interprofessional team, patient, family, or caregiver.
• Promotes respect, equity and empathy in interactions with diverse populations and advocates for patient-family centered continuity of care.
• Evaluates changes in patient’s condition and updates the plan of care in EMR in collaboration with family/c caregivers and the interprofessional team.
• This is a safety-sensitive position; ability to work in a constant state of alertness is essential.
