Job summary

Heartland Hospice is seeking a dedicated RN Case Manager to lead end-of-life care for patients in their homes or preferred settings across Illinois. You will guide families through the hospice journey with personalized care plans and compassionate nursing, ensuring timely assessments and high-quality support.

The role emphasizes collaboration with physicians, LPNs, CNAs, social workers, and chaplains to coordinate care and educate families.

Qualifications

• Requires an RN license in the state of employment.
• Minimum 1 year RN experience in acute care, hospice, home health, or related field preferred.

Responsibilities

• Lead patient assessments and ongoing reassessments, including documentation.
• Deliver skilled nursing care per Plan of Care in patients' homes or alternative settings.
• Coordinate with IDG to develop and refine care plans.
• Educate patients and caregivers on disease process, medications, and end-of-life care.
• Maintain accurate clinical documentation and ensure infection control compliance.
