Qualifications

• High School Diploma or GED required.
• 1+ year of Hospice experience
• 1+ year of Management or Leadership experience
• 1+ year of experience in Home Health Care, Epic Home Health, or Home Health
• Valid Registered Nurse license in Illinois (IL) required.
• Certifications: AHA BLS and ACLS.
• Willingness to travel across multiple counties.

Required Qualifications

• 1+ year of Hospice experience
• 1+ year of Management or Leadership experience
• 1+ year of experience in ANY of the following:
  • Home Health Care
  • Epic Home Health
  • Home Health
• Valid Registered Nurse (RN) license in the state of Illinois (IL)
• Valid Certifications:
  • American Heart Association Basic Life Support (AHA BLS)
  • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
• Comfortable with required travel across multiple counties

Overview

Key Responsibilities

• Deliver compassionate, patient‑centered care in home and facility environments

Schedule & Shift Details

• Monday–Friday | 8:00 AM – 5:00 PM(Start/end times may vary daily; availability required during these hours)
• Weekend & On‑Call Requirements:
  • One (1) 24‑hour weekend day per month
    • 8 AM–5 PM scheduled visits
    • 5 PM–8 AM true on‑call coverage
  • Two (2) weekday on‑call shifts per month
    • 5 PM–8 AM
  • Two (2) holidays per year
    • One summer holiday
    • One winter holiday
    • Plus three floating holidays
• Additional stipends and compensation provided for on‑call coverage
• Dedicated After‑Hours RN team support (three RNs on every night)

Travel Requirements

• Local travel required across multiple counties
• Must be comfortable driving within the assigned territory to visit patients in homes and facilities

Pay Transparency Compensation varies based on location, experience, and performance. Some sites require disclosure of pay and benefits. This role may also be eligible for bonuses or incentives depending on business needs. Employees may receive health insurance, optional dental and vision coverage, life and disability insurance, retirement savings plans, paid holidays, and PTO or sick time. The estimated hourly rate for this position is 80K
