Job summary

Gentiva in Illinois is seeking a Hospice RN / RN Case Manager to lead patient care at the home or other living environment. You will perform comprehensive assessments, implement and document plans of care, and collaborate with an interdisciplinary team to refine patient goals and comfort measures.

The role emphasizes education for patients and caregivers, timely documentation, and adherence to regulatory standards while delivering compassionate end-of-life support to families.

Qualifications

• Current license to practice nursing.
• Minimum of 1 year RN experience in hospice, home health, or related settings.
• CPR certification.
• Valid driver’s license and automobile insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments, providing education to patients, family members and caregivers.
• Deliver skilled nursing interventions and symptom management per goals of care.
• Develop and update individualized plans of care including visit frequency to meet needs.
• Coordinate care across interdisciplinary teams with physicians and nurse practitioners.
• Educate patients and caregivers on disease, self-care, end-of-life care, medications, nutrition and dietary needs.
• Document timely clinical records and maintain infection control standards.
• Support continuity of care by recognizing hospice referrals and transitions in levels of care.
