Job summary

Gentiva Health Services, Inc. is seeking a Hospice RN / RN Case Manager to lead patient‑centered care in home settings. You will perform comprehensive assessments, develop plans of care, and collaborate with an interdisciplinary team to provide symptom management and education to patients and families.

Qualifications

• RN license active in the state of employment.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, in person or by telecommunication.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care and visit scheduling.
• Coordinate care with an interdisciplinary team including physicians, LPNs, CNAs, social workers and chaplains.
• Educate patients and caregivers on disease process, self-care, end-of-life care, medications and nutrition.
