Job summary

Gentiva is seeking an experienced Hospice RN / RN Case Manager to lead patient assessments and provide skilled nursing care in patients’ homes. You will coordinate with an interdisciplinary team to tailor plans of care and support patients and families through end-of-life care.

The role emphasizes education, symptom management, and timely documentation while ensuring compliance and compassionate patient support across care settings.

Qualifications

• RN license in the state of employment
• At least 1 year of RN experience in acute care, hospice, home health, or related fields; hospice experience preferred
• CPR certification and valid driver’s license with auto insurance

Responsibilities

• Perform comprehensive RN assessments in home or facility settings
• Deliver and document skilled nursing care per Plan of Care
• Collaborate with IDG to develop and update patient care plans
• Educate patients and caregivers on disease process, medications, symptom management and end-of-life care
• Maintain clinical records and ensure infection control and compliance standards
