Qualifications

• Current RN license in the state of employment.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments in home or facility settings.
• Deliver skilled nursing care and manage symptoms per plan of care.
• Coordinate with an interdisciplinary team to refine patient care plans.
• Educate patients, families, and caregivers on disease, care techniques, and end-of-life options.
• Document clinical notes timely and maintain compliant records.
• Participate in on-call duties and field visits as required.
