Job summary

Gentiva is seeking a Patient Care Manager to lead hospice clinical operations, coordinate care, and develop staff while ensuring regulatory compliance. You will oversee patient care services, triage referrals, and guide IDG meetings to optimize patient outcomes in a compliant setting.

The role requires an active Illinois RN license, 3+ years of direct patient care including hospice/home health, and strong leadership with EMR proficiency.

Qualifications

• Active RN license in Illinois.
• Minimum 3 years direct patient care, including hospice/home-based care.
• Experience leading care teams and ensuring regulatory compliance.
• Knowledge of Medicare/Medicaid, JCAHO, ACHC standards.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritize clinical decisions.
• Triage incoming referrals and medical concerns promptly and compassionately.
• Lead IDG meetings, present updates, coordinate care plans, document minutes.
• Mentor staff and ensure compliance with policies and regulations.
• Ensure care delivery meets federal, state, local standards and company rules.
• Support performance improvement, quality assessments, and operational efficiency.
