Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead patient care in the Buffalo Grove, IL area. You will conduct assessments, develop and refine plans of care, and coordinate with an interdisciplinary team to deliver person-centered end-of-life care at home or in care settings.

You will educate patients and families, document nursing interventions, and participate in on-call duties while maintaining regulatory compliance and high-quality care.

Qualifications

• RN license required in state of employment.
• CPR Certification required.
• Hospice or palliative care knowledge preferred.
• Experience in home health, hospice, or palliative care.
• Strong communication with patients, families, and care teams.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN visits.
• Deliver nursing interventions and symptom management per plan of care.
• Develop and update individualized care plans including visit frequency.
• Coordinate care across interdisciplinary teams and physicians.
• Educate patients and caregivers on disease, end-of-life care, medications, and nutrition.
