Job summary

Advocate Health is seeking an RN Case Manager to join the Oshkosh Hospice team. You will coordinate care for terminally ill patients, educate families, and lead the IDG to implement patient care plans.

The role requires compassionate collaboration, nursing licensure, and ability to work with interdisciplinary teams. A sign-on bonus may be offered and a standard benefits package accompanies the position.

Qualifications

• Completion of an accredited or approved program in Nursing.
• Typically 1–2 years of nursing experience including home health, hospice, medical/surgical, long term care, emergency care/ICU or oncology.

Responsibilities

• Identify and coordinate patient/family care for terminally ill patients in hospice settings.
• Educate patients and families about disease processes, hospice philosophies, and signs of decline.
• Review caseload to ensure appropriate level of care and make referrals as needed.
• Lead the interdisciplinary care group (IDG) and develop/ revise the Plan of Care.
• Document in the EHR according to standards and policies.
• Assess patient response to treatments and update care plans as needed.
• Supervise Home Health Aide and Licensed Practical Nurse visits.
• Facilitate discharge planning and coordinate with community resources.
