Job summary

Advocate Health's Oshkosh Hospice team seeks an experienced Registered Nurse (RN) Case Manager to coordinate patient/family care across hospice settings and to guide end-of-life planning in line with agency standards.

The role emphasizes education, interdisciplinary collaboration, and ongoing assessment to maximize quality of life for patients and families. Schedule supports Monday–Friday, 8:00 a.m.–4:30 p.m., with rotating holidays and a $3,000 sign-on bonus for external candidates.

Qualifications

• Solid nursing knowledge and clinical assessment abilities.
• Excellent communication with patients, families, and the IDG.
• Ability to interpret complex patient data and adjust plans.
• Strong decision making in high-stress environments.
• Competent in coordinating multi-disciplinary care.

Responsibilities

• Identify and coordinate patient/family care across hospice settings.
• Educate patients and families on disease processes and end-of-life care.
• Review caseload to ensure appropriate services and levels of care.
• Administer medications and treatments as prescribed.
• Lead the IDG and develop the Plan of Care with team collaboration.
• Document accurately in the Electronic Health Record per policy.
• Assess patient responses and update care plans and orders.
• Supervise Home Health Aide and Licensed Practical Nurse visits.
• Coordinate discharge planning and post-discharge resources.
• Execute OASIS and hospice assessments when applicable in WI.
• Mentor new staff and ensure patient safety and safety protocols.
