Job summary

Advocate Health is seeking a Registered Nurse (RN) Case Manager to join the Oshkosh Hospice team. This full-time role emphasizes compassionate, patient-centered care across hospice settings with a schedule of Monday–Friday, 8:00 a.m.–4:30 p.m.

There is potential for coverage of neighboring teams as needed and a $3,000 sign-on bonus for external candidates. The role requires an active RN license, BLS, and a valid driver’s license, with 1–2 years in relevant nursing settings.

Qualifications

• Registered Nurse license issued by the state of practice.
• BLS for Healthcare Providers certification (AHA) required at hire.
• Valid driver's license required.
• Typically 1-2 years in nursing with home health, hospice, medical/surgical, LTC, emergency, oncology or related areas.

Responsibilities

• Identify and coordinate patient/family care to support terminally ill patients in hospice settings.
• Coordinate care with the IDG and develop/update the Plan of Care.
• Administer medications and treatments as prescribed by the physician.
• Enter, complete and update documentation in the EHR per agency standards.
• May conduct OASIS assessments for Palliative Care patients in Wisconsin.
