Job summary

Aurora Health Care is seeking an experienced registered nurse to coordinate end-of-life care for hospice patients in Oshkosh, Wisconsin. You will assess needs, educate families, and ensure comfort and high quality of life.

In addition to nursing care, you will document in the electronic health record, update care plans, supervise home health staff, and collaborate with the interdisciplinary team to provide consistent patient care across services.

Qualifications

• RN license issued by the state in which practicing.
• BLS for Healthcare Providers certification required on hire.
• Completion of an accredited nursing program.

Responsibilities

• Coordinate patient/family care to support terminally ill patients in hospice settings.
• Educate patients and families about disease processes, hospice philosophies, and signs of decline.
• Review patient caseload to ensure appropriate level of hospice care and referrals as needed.
• Administer prescribed medications and treatments.
• Lead the IDG for each patient and collaborate to implement the Plan of Care.
• Document in the electronic health record per standards and policies.
• Assess patient responses and update care plans accordingly, communicating changes to the team.
• Supervise Home Health Aide and LPN visits per standards.
• Assist with discharge planning and continuity of care post-discharge.
• Perform OASIS and related assessments where applicable in WI.
