Job summary

Advocate Health is seeking an RN Case Manager for the Oshkosh hospice program, 40 hours per week, Monday–Friday, 8:00 a.m.–4:30 p.m., with rotating holidays. The role requires compassionate, patient-centered care and collaboration with the interdisciplinary team.

Responsibilities include coordinating patient/family care, educating on disease trajectories, maintaining thorough documentation, and participating in discharge planning.

Qualifications

• Requires RN license in state of practice and BLS certification
• Typically 1–2 years nursing experience in home health, hospice, medical/surgical, LTC, emergency care/ICU or oncology
• Completion of an accredited nursing program
• Excellent communication and interpersonal skills required

Responsibilities

• Coordinate patient/family care for terminally ill patients across hospice settings with regular contacts
• Educate patients and families on disease processes, trajectory, hospice philosophies and end-of-life care
• Review caseload to ensure appropriate services and hospice level of care; refer to team members as needed
• Administer medications and treatments per physician orders
• Lead the IDG, develop and update the Plan of Care, and coordinate services with interdisciplinary team
• Document accurately in the Electronic Health Record and update as required
• Assess patient response to treatment and adjust care plans; communicate changes to physicians and IDG
• Supervise Home Health Aide and LPN visits; provide staff instruction per policy
• Coordinate discharge planning and post-discharge resources
• May conduct OASIS and hospice assessments in WI, and ensure continued skilled nursing/home-bound criteria
