Job summary

Advocate Health is seeking a Registered Nurse Case Manager to join the Oshkosh Hospice team in Wisconsin. The role focuses on compassionate, patient-centered end-of-life care, coordinating services and leading IDG discussions across hospice settings.

The position requires schedule flexibility and includes a 40-hour work week with a rotating holiday schedule. External candidates may qualify for a $3,000 sign-on bonus, with a pay range of $38.20–$57.30 per hour.

Responsibilities

• Identify and coordinate patient/family care for terminally ill patients in all hospice settings, with minimum contact every 15 days.
• Educate patients and families on disease processes, hospice care, and trajectory; update physicians per requirements.
• Review patient caseload to ensure appropriate services and care levels.
• Lead the interdisciplinary care group (IDG); develop and implement the Plan of Care and coordinate services.
• Enter and update documentation in the Electronic Health Record per standards.
• Document patient experiences with assessments and narrative entries.
• Assess patient response to treatments and update plans; communicate changes to physicians and IDG.
• Supervise Home Health Aide and licensed practical nurse visits per regulations.
• Facilitate discharge planning and coordinate post-discharge resources.
• WI ONLY: May conduct OASIS assessments for Palliative Care and establish plan of care.
