Qualifications

• RN license and BLS certification required.
• 1–2 years of nursing experience in home health, hospice, medical/surgical, or oncology preferred.
• Ability to lift up to 50 lbs and perform patient transfers.
• Demonstrated communication and documentation skills as part of care plans.

Responsibilities

• Identify and coordinate patient/family care in hospice settings.
• Educate patients and families on disease trajectory and hospice care.
• Review caseload to ensure appropriate level of care and services.
• Administer prescribed medications and treatments.
• Lead IDG and develop/implement patient plans of care.
• Document in electronic health record per standards and guidelines.
• Assess patient response and update care plans accordingly.
• Supervise Home Health Aide and LPN visits; mentor staff.
• Coordinate discharge planning and continuity of care after discharge.
