Job summary

Advocate Health in Wisconsin is seeking a registered nurse for hospice care across patients and families, coordinating care within the IDG and ensuring Plan of Care alignment with standards. The role includes assessing needs, education, and advocating for quality end-of-life care.

Qualifications

• RN license and state nursing program completion required.
• BLS for Healthcare Providers certification required at hire.
• 1–2 years of nursing experience, ideally in home health, hospice, or related fields.

Responsibilities

• Identify and coordinate patient/family care for terminally ill patients to maximize quality of life.
• Educate patients and families on disease processes, hospice philosophy, and end-of-life care.
• Lead the IDG to develop and implement the Plan of Care and coordinate services.
