Qualifications

• RN license issued by the state in which the team member practices.
• Completion of an accredited nursing program.
• Typically 1–2 years of nursing experience in home health, hospice, or related settings.

Responsibilities

• Identify and coordinate patient/family care for terminally ill patients in hospice settings.
• Educate patients and families on disease processes, management, and end-of-life signs.
• Review caseload to ensure appropriate services and care level are met.
