Job summary

Gentiva Hospice is seeking an experienced RN Case Manager to lead in delivering compassionate hospice and palliative care in patients’ homes and community settings. You will assess, plan, implement, and coordinate individualized care with an interdisciplinary team to ensure comfort and dignity for patients facing life-limiting illness.

Qualifications

• Current license to practice nursing in the state of employment
• Hospice or palliative care experience preferred
• Ability to work in patients' homes and community settings

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person/telephonic RN assessments and provide education to patients, family members and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Provide patient and caregiver education regarding disease, self-care, end-of-life care, medications, nutrition and dietary needs.
• Communicate changes in condition to appropriate providers and adjust care as needed.
• Support continuity of care by coordinating referrals and transitions between levels of care.
• Document timely clinical records on all patients and maintain knowledge of regulatory guidelines.
