Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead personalized end-of-life care in patients' homes or other settings. You will be the primary contact for patients and families, guiding them through the hospice journey with compassionate, evidence-based nursing care.

Join an interdisciplinary team to develop and update plans of care, educate caregivers, document visits, and ensure regulatory compliance while maintaining comfort and dignity for patients and families.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year RN experience; hospice or home health preferred.
• CPR certification and valid driver’s license.

Responsibilities

• Perform comprehensive patient assessments and reassessments with education for families as appropriate.
• Deliver skilled nursing interventions and symptom management per goals of care.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across interdisciplinary teams and physicians.
• Educate patient and caregiver on disease process, self-care, end-of-life care and medications.
• Document timely clinical records and ensure regulatory compliance.
• Participate in on-call and patient support activities as applicable.
