Job summary

Gentiva Hospice seeks a compassionate Hospice RN / RN Case Manager to guide patients and families through the hospice journey.

You will perform comprehensive home-based assessments, deliver skilled nursing care per the Plan of Care, and collaborate with physicians, social workers, and chaplains to refine personalized care plans. Strong communication and education for families are essential.

Qualifications

• Active RN license and CPR certification.
• Minimum 1 year RN experience in hospice, palliative or home health.
• Knowledge of hospice care principles and regulatory guidelines.
• Strong communication and education skills for patients and families.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic evaluations.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized care plans with visit frequencies.
• Coordinate care across IDG and communicate changes to the care team.
• Educate patients and caregivers on disease process, end-of-life care, medications, and nutrition.
• Document timely clinical records and maintain infection control compliance.
• Participate in on-call and patient support activities as applicable.
