Job summary

Gentiva Hospice seeks an experienced RN Case Manager to lead in-home hospice care, coordinating with patients, families and the IDG to deliver individualized care plans. You will perform assessments, manage symptoms, educate families, document care, and collaborate with physicians, chaplains, social workers, and other team members to ensure timely, compassionate end-of-life support.

Qualifications

• Current license to practice nursing in the state of employment.
• CPR certification required.
• Minimum of one year RN experience in acute care, hospice, home health, or related fields.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and re-assessments; educate patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized care plans including visit frequency.
• Coordinate care with physicians and interdisciplinary team members.
• Educate patients and caregivers about disease, self-care, and end-of-life care.
• Communicate changes in condition to the care team for timely interventions.
• Document timely clinical records and maintain compliance with regulations.
• Support continuity of care and transitions across levels of care.
• Participate in on-call and patient support activities as applicable.
