Job summary

Gentiva Hospice is seeking a compassionate RN Case Manager to lead patient care in the home setting. You will conduct assessments, implement individualized plans of care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to support patients and families living with life-limiting illness.

Qualifications

• Registered Nurse license required.
• Hospice or palliative care experience preferred.
• 1 year RN experience in relevant settings.
• CPR certification.
• Current automobile insurance and valid driver’s license.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing care and symptom management.
• Develop and update individualized plans of care.
• Coordinate care across interdisciplinary teams (IDG).
• Provide education to patients and caregivers on disease, self‑care, and end‑of‑life care.
• Document timely clinical records and maintain compliance.
