Job summary

Gentiva Hospice is seeking a compassionate Registered Nurse Case Manager to lead patient care in home settings. You will conduct comprehensive assessments, build individualized care plans, and coordinate with a full interdisciplinary team to ensure high-quality hospice services for patients and families.

You will educate patients and caregivers, manage symptoms, and document care with attention to regulatory standards.

Qualifications

• Must be a registered nurse with an active license in the state of employment.
• Minimum of 1 year of RN experience in acute care, hospice, home health, or related settings preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments in person or via telecommunication.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
