Job summary

Remarkable Hospice seeks a dedicated RN to administer hospice care in patients' homes or facilities. You will coordinate with a multidisciplinary team and assume responsibility for the care plan with physician orders, working independently.

Ideal candidates hold an active RN license and CPR certification, with 2+ years of clinical RN experience. Some travel to patient locations is required in this role.

Qualifications

• Graduate of an accredited school of nursing.
• Two years of RN experience in a clinical setting; hospice experience preferred.
• Current valid State RN license and CPR certification.
• Driver’s license and reliable transportation.

Responsibilities

• Administer hospice care for clients in their residence or facilities.
• Develop and implement Plan of Care with physician orders.
• Document patient status accurately and timely per policy.
• Collaborate with interdisciplinary team and educate patients/families.
• Participate in on-call rotation and comply with regulations.
