Job summary

Chapters Health System, Inc. is seeking an RN Case Manager to assess patient/family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative, and supportive home-based care to keep patients in their homes where possible.

The role requires an active RN license in Florida, at least one year of hospice/hospital experience, EMR/EHR proficiency, and valid driver’s license with reliable transportation. Strong communication and physical ability are essential.

Qualifications

• Current license as RN in the state where the employee will be working.
• Minimum of one year nursing experience in hospice or hospital preferred.
• Alzheimer's Disease and Related Dementias Training as required by Florida Dept. of Elder Affairs (PACE).
• Experience with EMR/EHR systems is preferred.
• Mobile Driver – valid driver’s license and auto insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to lift patients and equipment; physical endurance.
• Ability to communicate effectively in English (verbal and written).
• Able to visit participants in their homes for assessments.

Responsibilities

• Provides and manages direct care to patients and families as part of the Interdisciplinary Team (IDT).
• Coordinates the Plan of Care reflecting evolving patient needs.
• Educates patients and families about disease, prevention, and palliative care options.
• Performs home visits to assess safety, medication compliance, and DME needs.
• Communicates changes to IDT and orders to physicians.
• Develops relationships with case managers and facilities to coordinate referrals.
