Chapters Health System seeks an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team (IDT), and provide clinical, palliative and supportive care to patients in their home environment to maximize comfort and independence.

The role requires active RN license in Florida and at least 1 year of nursing experience in hospice or hospital settings, with EMR/EHR experience and a valid driver's license.

Qualifications

• Current nursing license in the state of Florida or as required
• Experience in hospice or hospital nursing preferred
• Experience with EMR/EHR systems
• Valid driver’s license and reliable transportation
• Strong communication in English (verbal and written)

Responsabilités

• Provide direct patient and family care as part of the Interdisciplinary Team (IDT)
• Coordinate and implement the Plan of Care reflecting patient needs
• Educate patients and families about disease process, palliative care, and safety practices
• Visit patients at home, assess needs, and ensure home safety and compliance
• Communicate with physicians, IDT members, and caregivers; SBAR communications
• Maintain documentation per policy and participate in quality improvement activities
