Job summary

Chapters Health System, Inc. is seeking a Registered Nurse, Case Manager to assess patient and family needs, coordinate the Plan of Care with the IDT, and provide clinical, palliative and supportive care to enable patients to remain at home where possible.

The role requires an active RN license in Florida, at least 1 year hospice or hospital experience, EMR/EHR proficiency, a valid driver’s license, and the ability to visit patients at home.

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• Experience with EMR/EHR systems.
• Valid driver's license and automobile insurance.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English (verbal and written).
• willingness to visit participants in their homes for assessments.
• Active BLS for healthcare professionals from AHA/Red Cross.

Responsibilities

• Provide direct care to patients and families as part of the IDT and deliver appropriate nursing interventions.
• Coordinate the Plan of Care with the Interdisciplinary Team to address evolving patient needs.
• Educate patients, families, and caregivers about disease processes, prevention, palliation, and safety.
• Report changes in patient condition to IDT and other health professionals.
• Participate in IDT to evaluate hospice referrals and admissions for appropriate level of care.
• Communicate clearly with physicians, staff, patients, and families; document using SBAR.
• Supervise care provided by CHWs and HHAs as required.
• May be required to report for emergencies to ensure continuity of services.
