Job summary

Chapters Health System is seeking an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical and palliative support to keep patients in their homes as long as possible. A valid RN license and at least 1 year hospice or hospital experience are preferred.

The role requires a driving license, reliable transportation, and proficiency with EMR/EHR systems.

Qualifications

• Current RN license in the state of work.
• Minimum 1 year nursing experience, hospice or hospital preferred.
• EMR/EHR experience.
• Mobile driver with valid license and auto insurance.
• Reliable transportation for visit schedule.
• Ability to communicate effectively in English.

Responsibilities

• Provide and manage direct patient/family care as part of the IDT.
• Coordinate Plan of Care reflecting patient needs.
• Educate patients and families about disease, care, and safety practices.
• Document patient interactions in EMR and coordinate with IDT.
• Attend IDT meetings and communicate with physicians and staff.
