Job summary

Qualifications

• Current RN license in the state where employed.
• At least 1 year nursing experience in hospice or hospital preferred.
• Experience with EMR/EHR systems and charting.
• Valid driver's license and reliable transportation.
• Ability to visit patients at home and perform assessments.

Responsibilities

• Provides direct care as part of the Interdisciplinary Team (IDT) with psychosocial, spiritual, cultural, physical and biological components.
• Coordinates the Plan of Care reflecting patient needs and evolving conditions.
• Educates patients, families and caregivers about disease, care options and safety.
• Communicates changes to IDT and physicians; participates in IDT meetings.
• Supervises Home Health Aides as requested.
