Job summary

Chapters Health System seeks an RN Case Manager to assess patient needs, coordinate the Plan of Care with the Interdisciplinary Team, and deliver clinical and palliative care to patients and families at home where possible.

The role requires an active RN license in the state, 1 year hospice/hospital experience, EMR/EHR skills, and a valid driver’s license with reliable transportation. Strong communication and safety awareness are essential.

Qualifications

RN license in the state of employment.
Minimum 1 year nursing experience in hospice or hospital setting preferred.
Experience with EMR/EHR systems.
Mobile driver with valid license and auto insurance.
Reliable transportation to meet visit schedule.
Ability to use equipment with sensory functions.
Fluent English verbal and written communication.
Ability to visit participants in their homes for assessments.
Ability to meet physical requirements incl. lifting.
Active BLS for healthcare professionals.

Responsibilities

Provide direct care to patients and families as part of the Interdisciplinary Team.
Coordinate Plan of Care reflecting patient’s evolving needs.
Educate patients and families about disease process, palliative care, safety and caregiving.
