Job summary

Qualifications

• RN license required in the state of employment.
• Minimum 1 year of nursing experience in hospice or hospital; Florida Alzheimer’s training optional.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance per policy.
• Reliable transportation to meet visit schedule.

Responsibilities

• Provide direct care to patients and families as part of the IDT.
• Coordinate Plan of Care reflecting patient needs.
• Educate patients and families about disease, palliative care, safety and caregiving.
• Communicate changes in patient condition to IDT.
• Participate in IDT meetings and referrals for level of care.
