Job summary

Chapters Health System, Inc. is seeking an experienced RN Case Manager in Florida to assess patient and family needs, coordinate Plan of Care with the Interdisciplinary Team, and provide clinical and palliative care at home to help patients stay in their own homes.

The role requires an active Florida RN license, at least 1 year of hospice or hospital nursing experience, strong communication and EMR skills, a valid driver’s license, reliable transportation, and BLS certification.

Qualifications

• Current RN license in the state where the employee will work.
• Minimum of one year nursing experience in hospice or hospital setting preferred.
• Active BLS certification for healthcare professionals.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English (verbal and written).
• Ability to visit patients in their homes for assessments.
• Ability to perform essential functions and physical requirements including lifting.

Responsibilities

• Provides and manages direct care to patients and families as part of Interdisciplinary Team (IDT).
• Coordinates the Plan of Care, ensuring an individualized Plan of Care is developed.
• Delivers clinical, palliative and supportive care to keep patients in their home environment.
• Documents patient/family contact information in the EMR and communicates with the IDT.
• Initiates and completes necessary nursing interventions to alleviate symptoms and maximize comfort.
