Qualifications

• Current RN license in Florida.
• Minimum of one year nursing experience in hospice or hospital preferred.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.

Responsibilities

• Provide and manages direct care to patients and families as part of Interdisciplinary Team (IDT).
• Coordinate the Plan of Care reflecting patient’s evolving needs.
• Educate patient, family, caregivers and other health professionals about disease process and palliation.
• Home visits to assess home safety and medication/nutritional compliance.
• Communicate with physicians and IDT members; SBAR as needed.
• Supervise patient care provided by Home Health Aides and CHWs as requested.
