Job summary

Chapters Health System is seeking an RN Case Manager to assess patient and family needs, coordinate care with the IDT, and provide clinical, palliative and supportive care to keep patients in their homes where possible.

The role requires an active RN license in the applicable state, at least one year of hospice or hospital experience, and proficiency with EMR systems. A valid driver’s license is required for home visits and scheduling flexibility.

Qualifications

• Current license as RN in the state where the employee will be working.
• Minimum of one year nursing experience in hospice or hospital settings preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance per Company policy.
• Coordinate home visits and patient assessments.
• Active BLS for healthcare professionals from AHA or Red Cross.

Responsibilities

• Provides and manages direct care to patients as part of the Interdisciplinary Team (IDT).
• Coordinates the Plan of Care reflecting patient needs.
• Educates patients and families about disease processes, prevention, palliative care, and safety practices.
• Conducts home visits to assess safety, medication compliance, and living conditions.
• Communicates with physicians and IDT members and documents in the EMR.
