Job summary

Anchor Health in California seeks an experienced Hospice RN Case Manager to join our team. You will assess patients and families, develop care plans, and coordinate services to support comfort and quality of life for patients at home.

This role requires a California RN license, CPR, and a valid CA driver license. Prior hospice or home health nursing experience is preferred, with collaboration across an interdisciplinary team.

Qualifications

• Graduate of accredited school of nursing.
• Current RN license in California.
• Current CPR Certificate.
• Two years nursing experience, or a BSN with RN licensure.
• Valid CA driver license and insured vehicle.
• Excellent observation, verbal and written communication skills.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provide complete physical assessment and history of current and previous illness.
• Conduct home visits and assess patient status.
• Initiate and revise the plan of care as patient needs change.
• Develop a care plan with goals based on nursing diagnoses and family involvement.
• Administer medications and treatments as prescribed.
• Attend and participate in Interdisciplinary Group Meetings.
• Complete, maintain, and submit accurate clinical notes.
• Supervise ancillary personnel and delegate tasks as required.
