Job summary

Anchor Health is seeking a Hospice RN Case Manager to coordinate end-of-life care in patients' homes and communities. The RN initiates and directs the hospice care plan, assesses patient needs, and collaborates with an interdisciplinary team to support comfort and quality of life.

Ideal candidates hold a CA RN license, CPR certification, have hospice experience, and possess strong interpersonal and communication skills to educate families and coordinate care across settings.

Qualifications

• Current CA RN license.
• Hospice care experience preferred (1 year).
• CPR certification required.
• Valid driver's license and reliable transportation.
• Excellent interpersonal skills and a passion for Hospice.

Responsibilities

• Assess and update the hospice care plan for each patient and family.
• Provide prescribed medications and symptom management.
• Coordinate care with the interdisciplinary team and physicians.
• Educate patients and families on care plans and self-care.
• Document clinical notes and patient responses to treatments.
• Ensure a safe home environment and continuity of care.
